The program manager will be responsible for supervising staff and overall operations for three programs: American House and Kimball Court Apartment buildings in St. Paul. The Case Manager will work with individuals and families residing in Dakota County, providing housing search and housing stability services in order to end or avoid episodes of homelessness and housing instability. The Dakota – Housing Services Case Manager will provide case management and care coordination services to individuals that have been referred directly from Dakota County Social Services due to homelessness or imminent risk of homelessness, or who are on a rental voucher requiring housing search and stability services. Case management services include, but are not limited to: housing search, service coordination with existing providers, referrals and technical support to assist residents in applying for other needed or wanted services and benefits. Case manager will ensure access to mainstream funding sources to pay for ongoing services, outreach to community agencies to develop strong working relationships, communication and advocacy with landlords, and file documentation for housing vouchers.

Job Title: Access Clinician – Chemical Health

The Access Clinician will conduct chemical health assessments with individuals accessing care at Avivo and facilitate admission to 245G programs. This role will ensure that all individuals receive access to appropriate services and assistance in gaining access services. This individual will work as part of the Avivo Access Team. Position requires a Chemical Health Professional that is either: Licensed Alcohol and Drug Counselor (LADC),. Or Dually Licensed: Licensed Alcohol and Drug Counselor (LADC) and Licensed Mental Health Professional (LICSW, LP, LMFT, and LPCC).
Must be licensed and in compliance with the appropriate licensure board. (i.e. Board of Behavioral Health & Therapy, Board of Social Work, Board of Marriage and Family Therapy, or another licensing board.)
